Well this is a really hard question and I think it would be hard to answer because they can all be pretty scary but I think Pennywise (the clown from the Stephen King Movie "IT") is pretty creepy as he can bring your scariest moments/fears to life....or so you think..... Hense my user name. Great question!

To this day IT and Candyman. I know they aren't really "monsters" but they still totally get to me. *chills*

I think the one that creeps me out to this day was this show that I seen as a kid - it was about a Scarecrow that came to life and got even..... Not sure if it really qualifies as a "Monster" - but it scared the dickens out of me and to this day - just thinking about it... the creeps!
I think Werewolves are pretty cool - but - I have a hard time picturing them as being totally animalistic - I think that they would maintain some of the human qualities and be a bit more cunning than they are portrayed as having.

Pennywise the Clown, and plain, old-fashioned "witches" (the type you'll find in Scooby Doo movies, I mean). Bubble, bubble...God, I always get shivers running up the spine when I think of that great film The Witches...or The Blair Witch Project...or Scooby Doo and the Witch's Ghost...or Hocus Pocus.
Oh, and Ol' Hacksaw Face (aka the Halloween Boy) from Norman Partridge's awesome novel Dark Harvest. If you haven't read that book...all I can say is that you gotta.
